Create a page with membership contract

I wonder if there is how to create a page with a subscription contract. For example, the user fills out a form with your data and at the end there is a term of membership. When he checkar the box he will accept the term, and automatically e-mail the completed data and the term of membership is sent to the site admin email. There is how to do?
